How to Create a Sign-Up Sheet in Microsoft Forms (2026)
A sign-up sheet lets people claim a slot — a time, a shift, a dish to bring, or a volunteer role. Microsoft Forms handles this with a Choice question for the options, plus name capture so you know who signed up for what. You can even cap how many people pick each option.
1. Create and Name the Form
- Go to forms.office.com and select New Form.
- Name it clearly — for example, “Bake Sale Volunteer Sign-Up.”
- Add a description with the date, location, or instructions.
2. Add Slots as a Choice Question
- Click + Add new > Choice.
- Enter the question (for example, “Which shift can you cover?”).
- Add one option per slot — “9–11am,” “11am–1pm,” “1–3pm,” and so on.
- Toggle Required so everyone picks a slot.
- Allow Multiple answers only if a person can claim more than one slot.
3. Collect Names
- Open More form settings (…) > Settings.
- To capture who signed up, restrict the form to people in my organization and turn on Record name.
- For public sign-ups, instead add a Text question titled “Your name” (and “Email” if needed) and mark it Required.
4. Cap Sign-Ups per Slot
- Open the Choice question’s More settings (…).
- Microsoft Forms can show how many picked each option in results, but to hard-limit a slot, turn on One response per person in Settings and monitor the Responses tab.
- For strict per-option caps, pair the form with a Microsoft Lists or Bookings page — Forms shows the running tally so you can close a slot once it’s full.
5. Share the Sheet
- Select Collect responses.
- Choose the audience — Anyone can respond for the public, or your organization for internal sign-ups.
- Share the link or QR code; the Responses tab shows who claimed each slot in real time.
6. Troubleshooting
I don’t know who signed up for which slot
Turn on Record name (internal forms) or add a required “Your name” text question (public forms).
Too many people picked the same slot
Forms doesn’t auto-close an option. Watch the Responses tab and update the option label to “(FULL)” or close the form when a slot fills.
People are signing up twice
Turn on One response per person in Settings so each signed-in respondent can only submit once.
I need to see counts per slot at a glance
Open Responses > View results; the Choice question shows a bar chart with the count for each slot.
